The recent publication of the National Institute for Health and Care Excellence (NICE) guideline Kidney cancer: diagnosis and management (NG256) includes recommendations relating to the use of 99mTc-sestamibi SPECT/CT in selected patients with indeterminate renal lesions. The guideline recommends considering 99mTc-sestamibi SPECT/CT following multiphasic CT or MRI where increased confidence that a lesion is oncocytic (including oncocytoma or chromophobe renal cell carcinoma) would change management, particularly where biopsy is not possible or is declined by the patient. For many BNMS members, this represents a positive recognition of the contribution that nuclear medicine can make within multidisciplinary uro-oncology services and highlights an emerging area of clinical practice where departments may increasingly be asked to support local pathways. As interest in renal sestamibi SPECT/CT develops, nuclear medicine departments may wish to review local expertise, imaging protocols and service availability, and engage with local urology and radiology colleagues regarding potential referral pathways and future service requirements. The British Nuclear Medicine Society (BNMS) remains committed to supporting members through education, dissemination of best practice, and collaboration with relevant stakeholders as experience in this area develops.
